As far as I can recall, the only reference to Cap being an Avenger was in the subtitle. No one throughout the film ever labels him as such. Also, the subtitle was originally added by Marvel at the time in order to give it a better chance at the box office, because the studio heads were not sure if just having it titled "Captain America" would appeal to international audiences. Thus it was given "The First Avenger" to strongly hint that tie in. In fact, all the present day scenes were supposed to be post-credits, but were edited in post as bookends in order to explicitly tie in to the Avengers.
Here's one way to look at it: Carol's call sign may have inspired the name of the team while Cap coming back from the dead retroactively made him the "first".

Just saw it. I really liked it. It's not top tier Marvel by any means but it was still great. Some questionable music choices, a few other issues, but Larson did just fine and I enjoyed it.
