DS9 established Betazed as being in the core of the Federation. It's presumable they are running around already (since the head of Starfleet Academy, a very senior position, was a Betazoid in the red-flapper era of Picard's youth, at bare minimum first contact is around this era). There's also a completely random hypothesis, but maybe there were plans once upon a time to incorporate more mental powers into the story, kind of an homage to "Where No Man Has Gone Before", but this didn't get rewritten (or maybe re-framed) with that tangential plot. As someone who deals with multiple identities, which admittedly is already different from this "surgical" method of implanting one, I don't actually see the "snap" as being all too weird. Identities will surface when they do, when they can't sit down, to stand up and shout "You're a liar!" no matter how bad decorum it might be at the time. Sometimes that surface is smart enough to keep its mouth shut, sometimes it's not. I see the possibility being open that Voq presumed he had high enough testosterone to control scarring his mind so terribly, but ultimately can't THE POWER OF WILL down his biological impulses when those emotional triggers are staring him in the face. This isn't stated of course, and I'd need to rewatch carefully to scrounge up all the supporting evidence for or against, but, I can see it being fridge logic, being generous.
Personally the only motivation, though, that really makes sense is the personal one. Michael killed his idol, and Voq being a religious fanatic, seeks honorable revenge. But without political power to command a ship, or even the freedom to travel Fed space, he goes to crazy extremes.

so, what happened to Mirror Sato? shouldn't her family still be in control of the Terran Empire? pretty sure she would almost certainly take it over with the Defiant.
