It may have actually been a literally misguided defensive missile from Ukraine. Things are a bit unclear. Either way, it wouldn't have happened if Russia hadn't attacked Ukraine, so the blame still should be put down at the feet of Putler.

Things frequently don't go according to plan in war - it's impossible for them to always do so. And mistakes will always be made. This doesn't even seem to be in the "whoops, we messed up there" category of mistake, it's a consequence of the uncertainties that are present in any action (i.e. "do this and there's an X% change of this result, Y% of that one.") Under those circumstances the responsibility lies with whoever is the cause of it all. So if you fire missiles at someone then you're responsible for whatever happens when they hit, and whatever consequences there are of a decent missile defence system that doesn't work according to plan every single time (and none ever will).

Nato thinks it's Ukraine (even if accidental) but Ukraine is denying it.

So most likely, either Nato actually thinks it's Russia and is trying to de-escalate the situation and not feel forced to start WWIII, or Ukraine is being intransigent.

It's probably accidental no matter whose it is. Neither side wants to admit their missile guidance is so poor that they hit the wrong country.
