Like the Nexus supposedly being unable to be approached by ship despite the fact that it clearly can be, Warp 10 can clearly be navigated because Tom finds a way back to Voyager. The episode is showing us one thing whilst telling us another.

- What is the only thing stopping them from using warp 10?
- Salamanders.
- What do they have at the end of the episode?
- A cure for salamanders.
- Literally nothing is stopping them from returning home.
The show should have ended here and there is no reason why it didn't in-universe.

Yeah, that's pretty much the first reaction of basically everyone who sees this episode. But: the cure for salamanders requires someone to be around to administer the cure. If the whole crew is turned into salamanders, then no one is left to do so. Now, you might say the Doctor should be an exception to this, but the thing is that Warp 10 evolves him into AM, and then he'll just be mutating and mutilating the crew even further to torture them for his own amusement. Even if it did somehow take the Doctor offline,t he mutation still takes long enough that they can consciously communicate to other parties for a while afterwards, and even if it didn't then a freaking post-it note stuck to the viewscreen saying to check out the last log entry and btdubs the lizards are the crew would be enough to let Starfleet know how to turn everyone back when Voyager blipped into Earth orbit without their towels. They used it twice and neither left them any closer to home. The first time the shuttle ended exactly where it started, the second time it was thee days at high warp in the wrong direction. Torres even says in the episode that once they break the barrier it's a matter of navigation, which makes it crazy to send a manned mission. And yet, didn't, in 100% of the cases tried. In one case, he ended up exactly where he started, which is so infinitessimally small a chance, that you can't put it in a number in any modern-day computer, if the destination was literally everywhere in the universe. No, Warp 10 doesn't work that way.

I still think it's crazy to do a manned mission if they didn't know, or at least have a good ides, where it would end up.

When I watch this episode, every time I think about how could I fix this episode with minimal effort.
The best I got is introduce crewman Jane Buffy who joins Paris on the test flight. She goes through the mutations and instead of 'evolving' it was in fact deadly. Just the body falling apart. She goes first because of some defect the mutations take advantage of. The Doctor is able to use Crewman Buffy's accelerated mutation to cure Paris at the last second as usual.
Also a very in front and center line from Torres about after post flight analysis that Paris and Buffy got supremely lucky that they came back. That if Voyager tried it or another shuttle tried it, they probably would end up anywhere in the universe.
Not ground breaking in anyway. Probably in the bed would have been still below average but I think would have been better than what we got.
