There are also people who relentlessly trolled Adele when she lost weight because that was somehow being a traitor to "real" women or some such crap.

We must ignore the extremes on both sides and aim for the middle ground. Too thin is dangerous, too fat is dangerous. Neither should be glorified.

The sad thing is though that corporations are often more sympathetic to this sort of thing nowadays than regular people. Look at actress who plays Tilly on DISCO. She wouldn't have been cast in Old Trek I promise you that. The major problem we now face for both boys and girls is Instagram and the easy availability of Photoshop. Everyone is now jacked, stacked or improbably flawless and kids lack the ability to understand the difference.
Disco era Trek has made an effort of bringing in a wider variety of women than Old Trek did. Disco more so than Picard or SNW though. Offhand I do not remember anyone like Tilly in Old Trek.

I cannot make the same observation about the men though. They seem to be still casting men in the same body type as before. It's not like they have a movie era Scotty sized man yet.

That is just me though.
Your typical Star Trek man starts well built and then is allowed to become chunky. And apart from the infamous behind the scenes stories of Garrett Wang and Robert Duncan McNeill when they put on a spare tyre, the production staff never really seemed to mind. Meanwhile the only women who were allowed to get larger in Old Trek were those that started beautiful and then reached the menopause.

There is definitely a way to go on both sides. I would say though that Starfleet being ''not a military but absolutely is a military despite what Gene says'' probably has fitness requirements. It wouldn't make much sense in-universe to go too far beyond Tilly's body shape unless for a specific reason. Scotty only started to pile it on a bit after he semi-retired in TWOK as an example. The whole plot of that film was that they were all being kicked upstairs to allow a crew of younger and fitter people to take over - which is actually kind of amusing when you take into account TUC and NEM but there you go.
There is definitely an unfortunate circumstance of weight fluctuations being much more noticeable on women pound for pound. Gaining 50 lbs of fat alone within one year, people said I looked more or less like the same figure.

It depends on how large physically you are though. Men are typically larger than women, so a 5' 6" woman who puts on 30 pounds will look far more noticeable than a man who is even just 4" taller. It also depends on how that weight is distrubuted. Some women look good even after gaining weight due to it being well distributed just like it can happen to men. But you could also just gain weight and pack it all in the abdomen and either get that beer belly or flabby stomach.

I assume there is a physical readiness test PRT in Starfleet. With their advanced medical tech, I doubt they just zap people and they instantly are in shape. So they have to work out because even a diet can do only so much. You got to be able to lift so much, run so far.

Tilly would be too large for real life militaries. She would have been put in either the 'fat camp' or shipped out of the service assuming they started out in the beginning thinner. 23rd century? She might be pushing it even then. I know the actress is in her 30's and that in real life militaries would have reduced PRT standards to reflect her age assuming Tilly is in her 30's too.
