Doing some Google-Fu, the song also appears at the end of "The Death of the Incredible Hulk" with Lou Ferrigno. The movie lists the song as "And Something Ends" by Chris Forde. Good luck finding it online though, I've only found a low quality bootleg-version off the soundtrack so far here: youtube.com/watch?v=uOTv6aegekY

The answer is correct. Someone knows the name of that song. Many people in the production staff knew the Name. Composer of that Song probably earnt a royalties cheque and knows the Name.

Okay let me rephrase the question then. Can someone on here please tell me the name of the song as well as the artist for the song chuck used in the review for the gift when Kes was leaving on the shuttle?

It's "And Something Ends", by Chris Forde and it's incredibly hard to find at all online. The only "good" copy I got by so far, can be found here: youtube.com/watch?v=uOTv6aegekY in a bootleg-soundtrack of "The Death of the Incredible Hulk" with Lou Ferrigno, which uses it as part of it's ending theme. I think Chuck may be the only source of a good copy of the song at this point in time and I would suggest contacting him personally.
